Sara is the Joint Care Home Programme Lead in Bromley, jointly appointed by the NHS SEL Integrated Care Board and London Borough of Bromley. Sara oversees the local care home programme and leads on the Enhanced Health in Care Homes workstream. Upon starting the role in 2020, joint working with St Christopher’s and other One Bromley partners to provide an integrated Covid-19 care home support offer achieved the 2021 MJ Achievement Award for Care and Health Integration.

Sara continues to strives for integrated personalised and proactive care that is closer to home for Bromley’s care home residents.
