Associate Care Practitioner 

Hours:  Part Time (22.5 per week – 0.6 FTE)

Your new role

We are looking for registered healthcare professionals from a Social Work, to join our multi-disciplinary team comprising Consultant doctors and nurses, therapists and support staff to work in our Single Point of Contact (SPoC) and outpatient service. We are looking to step away from a conventional model and utilise skills of other professionals to develop our offer to our patients reaching the end of their life.

Your role will be to conduct telephone assessments with patients and triage them to the appropriate part of the service. This might be to the Specialist Palliative Care Services based in the community, Bromley Care Coordination Service, Doctors, Wellbeing and Rehabilitation Service, Centre Services or Social Work Department. At the end of this assessment, and when needed, the team may decide to book a home visit or appointment in a clinic with the patient.

You will also be able to assess and support patients face to face in our outpatients department.

SPoC is open from 7.30am – 21.00pm each day Monday to Friday and from 9-17.00 on Saturdays and Sundays – we are flexible in our approach to working hours and location after a period of time.

Are you solutions focussed, goal oriented and a great team player. Close working relationships with the wider health economy including GPs and the Primary Care Networks in which they operate, Community Nurses and the third sector will be a key to the success of the service.

Are you able to undertake comprehensive assessments of patients with life limiting illness and palliative care needs, recognising who can best support their needs using a goal centred approach and formulate care plans and preferences for care including the patient and family and preferences

Additionally, it is just as important to us that you have the following skills;

  • Passionate about looking after people and driving positive change to support your patients and carers;
  • Ability to ‘go the extra mile’
  • Confident, influential and with excellent customer service skills;
  • Excellent communicator & able to use advanced clinical reasoning
  • Keen to develop and learn
  • Have a can do attitude
  • Possess a full driving licence and have access to their own vehicle.
