36 hours per week – 12-month Fixed Term Contract (Maternity Cover):
We have an exciting opportunity for a Social Work England registered social worker to utilise your skills within our dynamic multi-professional teams. You will form relationships with patients, their families and service providers to formulate an accurate assessment of need, in line with local and national policies.
Providing psychosocial support for adults with life limiting illnesses and their families, you will advocate for patients, carers and families around complex emotional and practical needs. This post does not have formal statutory authority although, knowledge of statute for protecting an adult or child is essential and liaison with statutory services is expected.
You will address the needs of people under our care, working on site or out in the community; facilitating transfers between care settings where appropriate.
To succeed in this role
You will have experience of carrying out social work assessments with adults and/or families, using strengths based and asset-based methodologies. You will also have had post qualifying social work experience of working in a statutory organisation in the UK and an understanding of adult social care systems and processes.
Knowledge and understanding of the impact of life limiting illness and disability upon individuals, their families and the resources available to them is essential.
