Salary Range: Group 16 (Equivalent to Band 7, £45,024-£50,806 inclusive of Inclusive of High Cost Area Allowance)

Your new role
The Single Point of Contact is an important team in St Christopher’s. It is the front door of our services and the engine room of care coordination for the people who use our services.
We are looking for an enthusiastic Clinical Nurse Specialist to join our spirited Single Point of Contact Team, working alongside the Consultant Nurse and medical Lead to provide strong management and leadership, to support the development, provision and sustainability of the Single Point of Contact. You will be responsible for contributing towards plans for the development of services, identifying priorities for action on a daily basis, building and supporting the teams, providing clinical expertise and advice and engaging with other key stakeholders.
As a Clinical Nurse Specialist, you will offer the patients and families your skills in symptom control, social, emotional and spiritual care, in addition to providing advice, support and education to the primary health care team and hospital staff where appropriate, as part of an interdisciplinary team.
In your role as the young adult and learning disability lead, you will be required to be the first line contact for all referrals where patients are aged between 18 and 30 years of age regardless of diagnosis and for those who have a learning disability, regardless of age or diagnosis.
You will also be required to develop a programme of training for the SPoC and wider community teams for these groups to include the differences in need, the need for effective communication with not only the patient but the family and to be able to identify which services would best meet their need.
To succeed in this role
You will be a compassionate nurse with proven experience in palliative care/oncology with relevant qualifications and experience of working at Band 6/ equivalent or above. You will possess excellent oral and written communication skills, with the ability to work efficiently in a disciplinary team and manage stressful situations. You will also have experience and/ or qualification related to working with young adults and people with a learning disability and will be able to build excellent relationships with the related external stakeholders.
