Your new role

The Single Point of Contact is an important team in St Christopher’s. It is the front door of our services and the engine room of care coordination for the people who use our services. We are passionate about using a multi professional approach to holistically supporting our patients and their carers.

We are looking for an Associate Care Practitioner with a social work background, to join our multi-disciplinary team comprising of Consultant doctors and nurses, therapists and support staff to work in our Single Point of Contact (SPoC). 

Your role will be to support our holistic service offer by adding your expertise as a social worker to telephone and video assessments with patients. This will include managing urgent concerns and issues including directing calls to the appropriate part of the service. This might be to the Specialist Palliative Care Services based in the community, Doctors, Wellbeing and Rehabilitation Service, Centre Services or Social Work Department.

The Single Point of Contact also takes all incoming calls from patients and carers already under the care of St Christopher's. 

SPoC is open from 7.30am – 21.00pm each day Monday to Friday and from 9.00-17.00 on Saturdays and Sundays – we are flexible in our approach to working hours and location after a period of time.

To succeed in this role

We are currently looking for a dynamic, committed and enthusiastic registered social workers who strive to deliver care that is empowering to both the patient and to those around them to enable them to be the best they can be.  

If you are;

  • able to work autonomously, but in the knowledge that you have an experienced and supportive multidisciplinary team behind you comprising a Consultant Doctor, a Consultant Nurse and a variety of healthcare professionals with a varied background?
  • Solutions focussed, goal oriented, a great team player and able to develop close working relationships with the wider health economy including GPs and the Primary Care Networks in which they operate, Community Nurses and the third sector will be a key to the success of the service.
  • able to undertake or willingness to develop skills in comprehensive assessments of patients with life limiting illness and palliative care needs, recognising who can best support their needs using a goal centred approach and formulate care plans and preferences for care including the patient and family and preferences
  • Passionate about looking after people and driving positive change to support your patients and carers;
  • Able to ‘go the extra mile’
  • Confident, influential and with excellent customer service skills;
  • Excellent communicator & able to use advanced clinical reasoning
  • Keen to develop and learn
  • Have a can do attitude
  • Possess a full driving licence and have access to their own vehicle.
