    Why we need your help

    We are extending our project enabling people who so wish, to remain in their own homes in the last 2-3 weeks of their life from the Bromley and Lewisham areas to cover the borough of Croydon. We hope this service will prevent people needing admission to hospital or the hospice in the last few days of their life. We also hope to support those people who are being discharged from hospital at the end of their lives to spend the last few days at home where this is their wish.

    We would like to increase our team of existing Patient and Family Support volunteers to join the Single Point of Contact team (SPoC) as community support rapid responders to help more people to stay at home at the end of their lives. In particular, those who live alone and those whose families feel that there is no option but to ask for an admission to the hospice. At the moment approximately 30% of our inpatients live alone.
